The checklists / templates should represent an overview of the way the exercise was played from the point of view of the organizers and of the control staff.
The checklists / templates should be handed out at the beginning of the exercise so that the feedback can be logged as the exercise progresses, not at the end as the persons involved will likely be less motivated at that point.
